ABSTRACT

INTRODUCTION: The prevalence of malnutrition is high among head and neck cancer (HNC) patients and negatively impacts their quality of life, treatment, and survival rates. OBJECTIVE: To identify preoperative nutritional variables capable of predicting postoperative complications in surgically treated HNC patients. METHODS: This was a prospective cohort study of HNC patients referred for surgery as initial treatment. RESULTS: Sixty patients were included in the study; they had a median age of 65.5 years, were mostly me n (85%), and most had low education levels (90%) and low household income (78.3%). The incidence rate of severe postoperative complications (grades II, III, IV, and V according to the Clavien-Dindo grading system) was found to be 50%. The nutritional variables under investigation, namely calf circumference (CC), triceps skinfold, body mass index, and adductor pollicis muscle thickness were shown to predict postoperative complications in HNC patients, especially CC, which was found to be an independent predictor of complications (OR=0.8; 95%CI: 0.65­0.96). Each 1-cm increase in calf circumference was associated with a 20% decrease in the risk of postoperative complications. CONCLUSION: Our findings show the nutritional variables studied are useful in the prognostic assessment of HNC surgery.

Abstract Introduction Surgery is one of the most frequently used options in the treatment of head and neck squamous cell carcinoma. In surgical patients, the use of arterial lactate to assess hypoxemia and severe inflammatory states is well-founded. However, there are few studies on its use in patients with head and neck squamous cell carcinoma. The aim of this study was to investigate whether the serum arterial lactate level on the 1st postoperative day would be a predictor of postoperative complications in head and neck squamous cell carcinoma surgeries. Methods This is a prospective cohort, which evaluated 44 adult patients of both genders, with HNSCC, who underwent surgery associated with monobloc neck dissection as an initial treatment. Patients were divided into two groups, according to the presence or absence of postoperative complications: with complication (Clavien-Dindo II-V) and without complications (Clavien-Dindo 0-I). Student's t-test and its variants were used to compare continuous data. Pearson's or Spearman's test was used to correlate the data and p values ​​<0.05 were considered statistically significant. Results A total of 59% of the patients (n = 26/44) developed postoperative complications. Serum lactate was significantly higher in the group with complications when compared to patients without complications, respectively 2.15 mmoL/L (1.10-3.90) and 1.59 mmoL/L (0.70-3.44); p = 0.03. The prognostic accuracy of arterial lactate was 69% (95% CI: 54%-82%; p = 0.03), estimated by the ROC curve. A cut-off >1.7 mmoL/L was identified, with a sensitivity of 65.38% and specificity of 66.67%. Conclusion Arterial lactate measured on the first postoperative day is a good predictor of postoperative complications in patients with head and neck squamous cell carcinoma.


Resumo Introdução A cirurgia é uma das opções mais usadas no tratamento do carcinoma epidermoide de cabeça e pescoço. Nos pacientes cirúrgicos, o uso do lactato arterial para avaliação de hipoxemia e de quadros inflamatórios graves é bem fundamentado. Entretanto, existem poucos estudos sobre o seu uso em pacientes com carcinoma epidermoide de cabeça e pescoço. O objetivo deste estudo foi investigar se o lactato arterial sérico no 1° dia de pós‐operatório seria um preditor de complicações pós‐operatórias nas cirurgias do carcinoma epidermoide de cabeça e pescoço. Método Trata‐se de uma coorte prospectiva, que avaliou 44 pacientes adultos, de ambos os gêneros, com carcinoma epidermoide de cabeça e pescoço, submetidos a cirurgia associada ao esvaziamento cervical em monobloco como tratamento inicial. Os pacientes foram divididos em dois grupos, segundo a presença ou não de complicações pós‐operatórias: complicados (Clavien‐Dindo II a V) e sem complicações (Clavien‐Dindo 0-I). Na comparação dos dados contínuos, foi usado o teste t de Student e as suas variantes. Na correlação dos dados, usou‐se o teste de Pearson ou Spearman. Valores de p inferiores a 0,05 (p < 0,05) foram considerados estatisticamente significativos. Resultados Dos pacientes, 59% (n = 26/44) desenvolveram complicações pós‐operatórias. O lactato sérico foi significantemente maior no grupo com complicações em relação aos pacientes sem complicações, respectivamente 2,15 mmoL/L (1,10-3,90) e 1,59 mmoL/L (0,70-3,44); p = 0,03. A acurácia prognóstica do lactato arterial foi de 69% (95% IC 54%-82%; p = 0,03), estimada pela curva ROC. Foi identificado um cut‐off> 1,7 mmoL/L, com sensibilidade de 65,38% e especificidade de 66,67%. Conclusão O lactato arterial do primeiro dia de pós‐operatório é um bom preditor de complicações pós‐operatórias nos pacientes com carcinoma epidermoide de cabeça e pescoço.

ABSTRACT

Background: The combination of doxorubicin (DOX) with paclitaxel (PTX) effectively treats breast cancer (BC). However, DOX-associated cardiotoxicity (CTX) is aggravated by the use of PTX. Consensus is lacking about which drug sequence involves the most CTX. Objectives: To evaluate whether DOX followed by PXT or the reverse sequence has the greatest cardiotoxic potential in the treatment of BC. Methods: Prospective study of women with primary BC who received four cycles of DOX and 12 infusions of PTX. Participants were divided into Group 1 (G1; PXT before DOX) and Group 2 (G2; DOX before PXT) at the discretion of the oncologist. CTX was defined as an absolute reduction in left ventricular ejection fraction (LVEF) > 10% to a value <53%. Patients underwentclinical evaluations and echocardiography before treatment (Phase 1) and one year after treatment (Phase 2). Results: Sixty-nine women were evaluated: 19 in G1 and 50 in G2. The groups had similar clinical characteristics. The doses of radiation, DOX, and PTX used were similar. Eight (11.6%) patients developed CTX: two (10.5%) in G1 and six (12.0%) in G2 (p=0.62). The mean LVEF was similar between groups in Phase 1 (G1=65.1±3.5%; G2=65.2±3.9%; p=0.96), with a significant reduction noted after one year in both groups: G1=61.4±8.1% (p=0.021) and G2=60.8±7.6% (p<0,001). Although lower, mean LVEF remained similar between groups after Phase 2 (p=0.79). Conclusions: In women with BC who underwent chemotherapy, the incidence of CTX at the end of the first year of treatment was similar regardless of whether DOX was used before or after PTX. (AU)

ABSTRACT

Antecedentes: En América Latina para mejorar el desempeño del sector salud y para frenar procesos de corrupción, se ha venido promoviendo el desarrollo de iniciativas de control, a través de ejercicios de rendición de cuentas por parte de los actores sociales involucrados en el sector salud. Objetivos: Describir y analizar la implementación de un proceso de control social a la gestión en salud con participación de usuarios de los servicios en Cali. Metodología: Se llevó a cabo en Cali una experiencia piloto de control social promovida por líderes comunitarios del sector salud, en la cual se usó como mecanismo la rendición de cuentas. El proceso se desarrolló en etapas, dando como resultado la realización de tres ejercicios públicos de rendición de cuentas. Además, con base en la experiencia, se diseñó una metodología para el desarrollo de iniciativas ciudadanas de rendición de cuentas. Resultados: Participaron indirectamente 745 personas y directamente 54 líderes comunitarios de las Asociaciones de Usuarios de las Empresas Sociales del Estado de la Red Pública de Salud de Cali, quienes contaron con apoyo técnico y financiero para promover dos de las tres rendiciones de cuentas realizadas. En cada una de las etapas se identificaron debilidades técnicas y metodológicas. Conclusiones: El proceso mostró que aunque se avanzó en comprender la importancia del control social en salud y los alcances de la rendición de cuentas, se identificaron obstáculos relacionados con las dificultades en el acceso a la información, bajo liderazgo comunitario en el tema de control social y resistencia de los funcionarios públicos a que su gestión sea objeto de control social.


Background: In Latin American for improving health sector performance and for decreasing corruption process, a social control initiatives like accountability process are been realized by social actors involved in health sector. Objectives: To describe and to analyze accountability to health management process with the involvement of health services consumers. Methodology: A pilot control social experience was developed in Cali with community participation in an accountability experience. Three public accountability experiences were developed. A methodology proposal was design to achieve community participation in accountability process. Results: 745 persons, indirectly, and 54 community leaders of Consumer Association of health local services of Cali, directly, were wrapped up. They had technical support in two out of the three accountability meetings carried out. In each stage was identified technical and methodological weakness in the accountability process. Conclusions: Despite of an understanding of the accountability process in health was improved, the experience showed some obstacles such as: difficulties in the access of information, low community leadership in social control issue and resistance of public health workforce to see their performance in health sector as a matter to be account.

ABSTRACT

A Fibromatose Gengival Hereditária (FGH) é descrita como uma condição bucal rara, clinicamente manifestada por um crescimento lento, progressivo, difuso e benigno dos tecidos gengivais. Alguns autores defendem a hipótese de o aumento gengival ser transmitido por um gene autossômico dominante, porém outros mostram que a FGH parece ser transmitida por um gene autossômico recessivo, em que pessoas não afetadas transmitemessa anormalidade a seus descendentes. O modo de transmissão hereditária parece ser autossômicodominante, relacionado ao cromossomo 2p21. A FGH pode se manifestar, de um modo geral, em duas situações diferentes: a primeira, quando o crescimento exagerado da gengiva constitui a única alteração patológica clinicamente observável, e a segunda, quando a fibromatose gengival é uma das alterações que se manifesta associada a outras patologias sistêmicas, como o retardo mental, a epilepsia, a hipertricose e a surdez progressiva. Hereditary gingival fibromatosis (HGF) is described as a rare oral condition, clinically manifested by a slow, progressive and benign enlargement of gingival tissues. Some authors support the hypothesis that HGF is transmitted by an autosomal dominant gene, while others think that HGF seems to be transmitted by an autosomal recessive gene, when non-affected subjects transmit this abnormality to their descendants. Thehereditary transmission mode seems to be an autosomal dominant inheritance related to the chromosome 2p21. In general, HGF can be observed in two different clinical situations: the first, when the exaggerated growth of the gingiva constitutes the only clinical pathological alteration observable and the second, when gingival fibromatosis associates with other systemic conditions, such as mental retardation, epilepsy, hypertrichosis and gradual deafness. The aim of this study was to review the literature on hereditary gingival fibromatosis and describe its characteristics to enable the diagnosis, treatment and control of this disease

ABSTRACT

Epstein-Barr virus (EBV)-encoded small RNA can be detected in about 1-17 % of gastric carcinomas. To elucidate lifestyles and other factors related to such an EBV-associated gastric carcinoma (EBV-GC), we conducted a case-control study in Cali, Colombia. The study subjects were 368 patients with gastric carcinoma newly diagnosed during the period between September 2000 and June 2003, including 42 EBV-GC cases. We obtained information on lifestyles, dietary habits, and occupational exposure by a questionnaire. The frequency of EBV-GC was related to birth order of patients (P for trend =0.025). More precisely, EBV-GC was much less frequent among the patients who were the eldest child in a family (P=0.007). Those findings were contrary to what was reported by the study conducted in Japan, where EBV-GC was more frequently observed among eldest brothers/sisters. A possible explanation for the apparently conflicting results is that EBV-GC risk is related to the age at first EBV infection but its relationship is not monotonic. In addition to the relationship with birth order, the present study showed that high salt intake and metal dust exposure may be related to EBV-GC as reported by the Japanese study although these associations observed in the present study were not statistically significant. No significant association was observed in other factors, including dietary habits. Further studies seem warranted to elucidate the difference between Japan and Colombia with respect to the environmental factors related to EBV-GC cases.

ABSTRACT

Los problemas ambientales globales tales como los generados por la contaminación pueden ser considerados como variables económicas y por tanto deben ser analizados en términos económicos. La contaminación se conceptualiza económicamente como una externalidad negativa generada por los procesos de producción y de consumo. El presente trabajo tiene como objetivos: a) conceptualizar económicamente la contaminación, b) determinar su nivel óptimo, tanto para los diferentes agentes económicos implicados como para la sociedad en su conjunto, y c) presentar varios métodos para alcanzar este nivel. La contaminación implica un costo, siempre que el agente económico que la sufre no sea compensado por el agente que la genera. En este sentido, el nivel socialmente óptimo de contaminación está determinado por la intersección de las curvas del costo marginal del agente que sufre la contaminación y el beneficio marginal del agente económico que la genera. El nivel óptimo de contaminación puede ser estimado por varios métodos, tales como: el método impositivo descrito por Pigou y la libre negociación entre las partes descrito por Coase. Otras políticas ambientales utilizadas para alcanzar el nivel óptimo de contaminación son: a) la fijación de estándares ambientales, estableciendo la cantidad máxima de contaminación que se permite al agente contaminante; b) la fijación de subsidios para reducir la contaminación, estimulando al agente contaminante para que invierta en tecnologías limpias; y c) la emisión de permisos de contaminación, entre otros
